If you are seeking permission to use any Amy Wells Designs trademarks, logos, screen shots, copyrighted designs, or other brand features from the Amy Wells Designs Website (http://amywellsdesigns.co.uk), please provide the following information to allow us to review your request:
· registered company name of the organization and/or person requesting permission;
· registered company office address and other contact information (e-mail, phone, fax);
· country where the company is registered;
· company registration number;
· name of the book or website where Amy Wells Designs material will appear;
· exact Amy Wells Designs logos or other Amy Wells Designs materials to be used;
